Introduction

This document provides examples of PhD dissertation proposals that will advance research in paediatric home health care and virtual childcare for families of children with complex health issues. These contribute to several important foci, including enhancing clinical outcomes, reducing access inequities, developing digital care models, assisting family caregivers, and strengthening workforce education and training. Each of the topics aims to provide evidence and practical solutions that lead to improved child and family wellbeing and eventual policy and practice in home and virtual care.

Paediatric Home Health Care Research Topics

1. Clinical Effectiveness of Paediatric Home Health Care: Impacts on Child and Family Health

Focus: This project will systematically review the clinical effectiveness of home health care (HHC) interventions on child health outcomes, including functional status, and the physical, mental, and financial health of family carers. The dissertation will identify the most important determinant(s) in the most significant change in child participation and family quality of life.

2. Access and Disparities in Paediatric Home Health Care: Patterns and Trends

Focus: To examine trends in the use of HHC and additional differences in use according to region, race/ethnic identity, language, and socio-economic status among children with disabilities and medical complexities. The dissertation will describe inequities in allocation, timeliness, and delivery of HHC services as well as recommendations for equitable policies.

3. Innovative Models of Care in Paediatric HHC: Integration of Digital Tools and Remote Monitoring

Focus: Develop and test model(s) for telehealth with remote patient monitoring, digital clinical decision support tools, and virtual interprofessional team coordination to manage chronic symptoms and prevent acute care episodes for children receiving HHC.

4. Education and Workforce in Paediatric Home Health Care: Training and Policy Approaches

Focus: To examine best practices for education, retention, and skill assessment for HHC providers and family carers, including the utilisation of virtual training simulations and peer-to-peer modalities. Also, to examine economic and policy system levers for expanding and sustaining the HHC workforce.

5. Measurement and Data Sharing in Paediatric Home Health Care: Improving Research and Quality

Focus: To develop & validate reproducible measures of access, quality & safety in paediatric HHC (e.g., adverse event detection & benchmarking). This project will propose models for data-sharing consortia and national patient registries to advance collaborative research and policies.

Virtual Child Care Topics

6. Integrated Virtual Care for Caregivers: Addressing Mental and Physical Health Needs

Focus: To design and evaluate the expansion of virtual interventions that provide mental health and physical health strategies to carers of children with ongoing health conditions that also assess impact on carer well-being, capacity, and child health.

7. Family-Centred Virtual Care Models: Enhancing Dyadic and Sibling Support and Care in Chronic Illness in Children

Focus: To co-design and test family virtual interventions that engage family systems (i.e., dyads of parents and their children, siblings) to advance family processes, relieve burden in carers, and enhance functioning in children. The project will identify best practices for co-design plans that include family systems.

8. Practical Supports in Digital Care: Financial, Respite, and Employment Assistance for Caregivers

Focus: To examine feasibility and impact related to embedding practical supports (e.g., respite care; financial counselling) onto virtual platforms for carers of children with complex needs. Research will examine outcomes on carer stress, economic position and sustainability of caregiving.

9. Equity in Technology-based Care for Carers: Co-Designing Inclusive and Accessible Solutions

Focus: To establish equitable access points to virtual care in carer populations that experience inequities. The dissertation will co-design with diverse carer populations to create culturally relevant, language-accessible, and flexible support mechanisms via digital technologies for carers and/or children that reduce care access gaps.

10. Lived Experience Partnerships in Digital Health: Impact on Relevance and Sustainability

Focus: To assess the extent to which co-researching with carers shapes the design and implementation of virtual care for carers. This project will create a framework for how lived experience expertise can be incorporated into digital health research and policy frameworks.
